Upload Dual Enrollment Admissions Documents

The Dual Enrollment Process has many steps for Admissions. Once you have completed STEP 1, the college’s online Dual Enrollment Application, the following documents are required for Admissions by the applicable Dual Enrollment document deadlines. After the college’s online Dual Enrollment Application is processed, you will need to obtain your Chatt Tech student ID number (900#) and securely upload the additional documents required for admission.

PLEASE allow up to 7 business days for documents to process and post to your Admissions file.

If the document upload form will not submit or you receive an error, please email your documents to dual.enroll@chattahoocheetech.edu. You must include the following in your email so we can match your documents to your records: your full name (first and last), date of birth, Chatt Tech Student ID# (900#), high school name and your email address.

Dual Enrollment Advisement Plan

This document must have 3 signatures to be accepted: counselor, parent/guardian, and student. Download the Dual Enrollment Advisement Plan Form here.

Verification of Lawful Presence

Student must upload a verification document. (EX: Birth Certificate, driver’s license/permit, etc.) If your driver’s license expires prior to the start of the term you are applying for, it will NOT be accepted. We recommend using your Birth Certificate, as this never expires.

Please provide your social security card only if you have been asked to verify your social security number. This document is not acceptable for Verification of Lawful Presence.

Qualifying Test Scores for Placement (Optional)

10th grade students must submit qualifying ACT or SAT scores to be eligible for core academic classes. 10th graders wishing to take core classes must email a copy of qualifying ACT or SAT scores to GaFutures. While not required for admission, placement testing is still available to applicants who need guidance on determining which coursework is appropriate for their skill level. If you are interested in AP credit, please use the above upload link to submit a copy of your AP scores for review. Please be sure the copy from CollegeBoard has your full name on the submitted document.
