Commercial Truck Driving Application Steps

Below are the steps for the CDL Program, please complete steps in the following order:

  1. Once you are within 60 days of the cohort start date, submit your certified 7 year Motor Vehicle Report for pre-approval before applying to the CDL program:
    • Certified 7-year Motor Vehicle Report (MVR)must be submitted using the CDL MVR Upload Form. The certified 7-year Motor Vehicle Report (MVR) must be acquired in person, at a Georgia Department of Driver Services facility. Reports must not include any moving or non-moving violations. Serious Georgia Commercial Disqualifications must include a reinstatement date or be completely cleared from the Certified Motor Vehicle Report. All certified MVR reports are reviewed for approval by college personnel.
    • Earliest Day to get MVR:
      • Fall August 18 (day class) = MVR June 20
      • Fall August 18 (night class) = MVR June 20
      • Fall October 13 (day class) = MVR August 15
  1. After your MVR has been approved, via email, please obtain your DOT Physical:
    • DOT Physicals must be obtained prior to obtaining the CDL Class A Learner’s Permit since the DOT Physical is required to be attached to the CDL Class A Learner’s Permit, with a new law put in place June 2025. The DOT Physical facility of your choosing will submit your results to the DSS via FMCSA online.
      • DOT Physicals must be valid until the final day of your CDL cohort. We recommend using our approved facility (Lake City Chiropractic) to obtain your DOT physical. Lake City is 2 miles from the North Metro campus and will electronically send your results to Chatt Tech.
      • If another facility is used or you already have your physical completed, the long form (form MCSA-5875, all pages) and medical card must be emailed to CDL@ChattahoocheeTech.edu.
  1. Complete and submit the Commercial Truck Driving Application Form once your certified 7 year MVR has been approved and you have received your Class A Learner’s Permit.
    • The application form requires an upload of TWO items:
      • Current unexpired Georgia Driver’s License. Copies of temporary paper licenses will not be accepted.
      • CDL Class A Learner’s Permit. Your permit can be obtained at any DMV office and it must be valid until the final date of your cohort. Study materials can be acquired at your local Georgia Department of Driver Services.  Permit testing will cover general knowledge, air brakes, and combination vehicle.
  1. Once you have completed the steps above, you will be contacted via email with details on how to schedule your drug test appointment with our approved facility – Lake City Chiropractic:
    • FMCSA Drug Test– will be sent to the CDL Admissions office by Lake City Chiropractic once completed.
    • Admission to the CDL program is first come, first serve basis. If a class fills prior to completion of your admissions process, you will be notified about the next possible class session.
    • Earliest Day to Drug Test:
      • Fall August 18 (day class) = Drug Test: July 21
      • Fall August 18 (night class) = Drug Test: July 21
      • Fall October 13 (day class) = Drug Test: September 15
  2. Complete the GSFAPPS application to apply for financial aid including the HOPE GRANT and HOPE CAREER GRANT. You can complete the GSFAPPS application at any time during the application process but they will not be able to see what you are qualified for until you have been admitted to the college and are registered for the classes. Please allow up to seven business days to process your financial aid application once registered.
  3. The first 25 (day) and 16 (night) students per cohort date to complete all items about will be admitted to the Chatt Tech’s Commercial Truck Driving Program. Once registered for your classes, you will be notified to make your payment in your Admissions Letter.
