Info Page Header Graphic

Documentation of Disability

At Chattahoochee Technical College, it is the responsibility of the student to provide documentation of his/her disability. Students may be referred to agencies outside the college to gain the necessary documentation of their disability. Once a referral is made, it is the student’s responsibility to complete the necessary steps to obtain this information.

Students who have physical/emotional disabilities or learning challenges may benefit from reasonable accommodations and/or assistance in the classroom. To request accommodations, equipment, or other services students must provide current professional documentation describing the disability and appropriate accommodations. Disabilities may be temporary or permanent, and can include: physical impairments: (i.e. visual, hearing, mobility, chronic illness, speech), psychological impairments (i.e. depression, bipolar disorder, anxiety, schizophrenia, OCD), or learning disorders (i.e. learning disabilities, acquired brain injuries, ADD, ADHD).

Confidentiality

Chattahoochee Technical College & Disability Support Services adheres to all state and federal laws concerning confidentiality. As an enrolled student, you are considered an adult and all information provided becomes confidential. Unless the student has signed the confidential disclosure statement, we will be unable to provide information to parents or other interested parties.  Our department goes through great lengths to protect the confidentiality of students & all sensitive documents.

Documentation Guidelines

Current diagnostic and evaluative reports completed by a trained professional are required as documentation. Documentation must:

  • Be on an official letterhead
  • Be signed by a qualified professional
  • Establish how you are impacted by disability

All evaluations, test results, and medical records are confidential and are used for the sole purpose of determining eligibility for accommodations. The nature of the disability or temporary disabling condition is not released to any other party except with the written consent of the student. There is no disclosure to other college personnel.

Forms for documenting various disabilities:

All documentation received by Disability Support Services is secured and considered confidential. The specific disability is not disclosed to anyone without the written permission of the student. Students may wish to complete a FERPA release form, so that information might be shared with parents and vocational rehabilitation counselors if necessary.

Documentation should indicate how the disability should be accommodated within reasonable means. Those recommendations will be taken into consideration when an accommodation plan is determined during intake. Students should be aware that accommodations only change the means in which information is provided. Standards set down by Chattahoochee Technical College or by TCSG cannot be waived or changed.

Disabilities include, but are not limited to: attention deficit disorder, brain injury or neurological impairment, autism and related disorders, communication disorders, chronic illness and medical impairments, hearing impairments, learning disabilities, motor or mobility impairments, psychological or psychiatric impairments or visual impairments. More information on disability documentation, services, technology and accommodations an be discussed with the Disability Support Services staff.
