Documentation Required to Apply

All documentation received by Disability Services is secured and considered confidential. The specific disability is not disclosed to anyone without the written permission of the student. Documentation may indicate how the disability should be accommodated within reasonable means. Those recommendations will be taken into consideration when an accommodation plan is determined during intake. Students should be aware that accommodations only change the means in which information is provided. Standards set down by Chattahoochee Technical College or by TCSG cannot be waived or changed.

Students with Attention Deficit/Hyperactivity Disorder need documentation that is no more than one (1) year old and that is signed by an individual with the credentials to make the diagnosis. If a psychological evaluation is used, it should be no more than three (3) years old.

Students with Learning Disabilities need a psychological evaluation that is no more than five (5) years old and or adult –normed and that is signed by an individual with the credentials to make the diagnosis.

Students with Psychological/Psychiatric Disabilities will need documentation that is no more than one (1) year old and that is signed by an individual with the credentials to make the diagnosis.

Students with a Physical or Mental Disability will need a report that is no more than five (5) years old and that is signed by an individual with the credentials to make the diagnosis.
